Your grade will include group leader self- assessment form

Parameters: Groups must be of a Therapeutic, Educational, or Psycho-educational nature.

Materials: You will turn in your materials the day that you present your presentation:
1) An informed consent form for group work
2) A copy of Group Rules/Behavior Expectations (what you would expect your group
to follow)
3) Your Group Planning Form
4) Copies of any handouts distributed during the group
5) Informational Flyer/Advertising Material for your group
