Undertaking mental health assessment is an important aspect of the role of mental health nurses. Mental health assessments need to be objective, person-centred with a degree of accuracy. It helps inform treatment options as well as an appropriate recovery plan.

This assessment task requires you to choose a consumer you are working with or have worked with in the past and write a comprehensive assessment that incorporates evidenced based practice. Assessments often relies on static and dynamic data-which incorporates the mental state examination (MSE) and corresponding Risk Assessment. The focus of this essay would be the MSE and Risk Assessment as well as the resulting recovery plan.

Section 1: Mental Status Examination (MSE) & Risk Assessment

1. It is absolutely important that you de-identify the consumer you are presenting. Use pseudonym and state this explicitly. If you have obtained permission from the consumer to write about them, please state so.
   
   
2. Each component of the MSE provides valuable information to the Clinician to best work out what the issues related to the consumer are. Given that elements of the MSE cuts across various disorders indicate why this is important to the particular consumer. Use the components listed below to guide your write up.
   
   
3. For each component of the MSE that you are presenting identify why it is significant in relation to the consumer’s diagnosis. Example, a consumer presents with a diagnosis of major depressive disorder, what is the significance of assessing their mood or affect? Provide evidence to support your findings.
   
   
4. For each of the component draw upon the literature that supports the assessment and that points to the variance that may exist. Indicate how you assessed each component i.e. direct or indirect observation, use of closed or open-ended questioning or the use of a specific assessment tool.

5. There is a strong link between the MSE and the Risk Assessment. Following your MSE you should also formulate a risk assessment.

Section 2: Recovery Focused Care Plan:

The second part of your write up/clinical report is the formulation of a recovery focused care plan targeting at least three areas of recovery.

1. Write 3 Mental Health Nursing Interventions for the Consumer you have assessed in the first part of the Assessment.

2. For each of these 3 MH Nursing Interventions you need to: Describe clearly a specific and detailed MH Nursing intervention supporting this description with in-text literature. Discuss how you would deliver the intervention for the consumer; this requires you to link your intervention back to the care of the consumer you chose. Consider how/when you might undertake the intervention and who else it might involve.
   
   
3. You need to make sure that your 3 MH Nursing Interventions are distinct and different from each other. Avoid interventions that essentially involve the same actions / components just described differently.
   
   
4. You will need to demonstrate how you have taken into consideration relevant principles of the Recovery Process (supported with in-text references).
