WORK PLACEMENT PROJECT

You are to develop and implement a community support plan for two individuals with a disability as part of your work placement. For each individual, students will need to identify needs, strengths and abilities, have researched and networked with approved community services and programs, organised required resources, supported the individual, the rest of the team, and performed a risk assessment.

Instructions:

During this task you need to do the following:

  • Identify the individual’s strengths, interests, abilities and support requirements
  • Determine appropriate services, resources and community opportunities
  • Work with the individual, their family and/or carer to develop and implement most suitable strategies
  • Recognise communication challenges and develop solutions to meet the individual’s communication needs
  • Identify challenges and barriers to participation and develop ways to overcome these challenges
  • Complete risk assessments and develop solutions to ensure activities, services and programs are suitable for the individual
  • Coordinate required logistics to support the individual participating in community services and programs
  • Monitor the success of strategies by reviewing the progress and response of the individual

To do this you must:

  • Identify two individuals with a disability at your work placement. Ask your supervisor to help you to identify suitable individuals. You will need to obtain consent from each individual to include them in this project. A permission form has been provided for this purpose.
  • For each individual you must document your support strategies. Support strategies must include details of the client’s needs, support requirements, resources required and community services. Use the template provided to record your plan.
  • Complete the questions included within the template for each individual
  • Complete a risk assessment for each individual

The project template consists of five sections:

  1. Identify client needs
  2. Determine social and community networks
  3. Identify required resources
  4. Communication and support
  5. Perform a risk assessment

Note: You must maintain the confidentiality of clients, family members and members of the support team. Do not include any identifying information in your assessment. Identify to clients by Client 1, Client 2 etc, or you may use first names only where these will not identify them.

Answers to the questions in this task will vary depending on the student experience of supporting different individuals during their work placement. The student should submit three completed reports as per the project outline provided.

WORKPLACE SUPERVISOR THIRD PARTY REPORT

Task Summary:

You will need to complete this task in your work placement service in the presence of your assessor. There are two parts to this task:

  • Part A requires you to provide support services to a client
  • Part B requires you to complete and store reports and documentation as per their work placement’s policies and procedures.

  • If your assessor sees that you have not shown appropriate skills or knowledge during any of these tasks, they will give you some feedback and you will need to do the specific task again.

You must obtain permission from your work placement supervisor to have this client participate in the task – see the permission form provided at the end of this task.

WORKPLACE SUPERVISOR THIRD PARTY REPORT

  • For this part of the assessment your workplace supervisor will observe you providing support services to a client according to their individual support plan.
  • The support that you provide your client will vary depending on the client’s needs. Ideally the work will involve the use of some equipment.
  • Following this observation by your workplace supervisor, your assessor will also ask you some verbal questions.

For example, discussing tasks with the team; sharing information; asking for advice; following instructions; reporting observations.

Respect the client, family and/or carer

For example, respecting differences; being polite and courteous; by being non-judgmental; treating everyone as individuals and so on.

Seek assistance for tasks outside the scope of own role

For example, when a situation occurs that you don’t know how to handle; if a client has a need that you are unable to address; a client asks for advice that you are not qualified to give, etc

Complete and maintain documentation according to policy, procedures and protocols

For example, writing a clear and concise report of observations; maintaining confidentiality/privacy; using correct terminology and so on.

Report observations to your supervisor

For example, providing a concise and accurate account of observations; answering any questions and so on.

Store information according to policy and protocols

For example, by filing documents in correct place; by transferring information onto a computerised record; by maintaining confidentiality (such as locking filing cabinets; not leaving passwords lying around; not leaving client documentation where others can view it, and so on).
