Relatives and Advocates will be present at the meeting. At this particular meeting the team discusses a 60 year old male who is not eating and who is depressed. This service user is visually impaired, suffers from dementia and has loss of motivation and often exhibits challenging behaviour.

• Discuss the different professional relationships that exist in the context of the case scenario in relation to the service user.

Discussion of the different professional relationship existing within the case

It is very important to establish the perfect relationship with the patients in order to understand their exact problem and to provide treatment as per their needs. Patient-physician relationship helps in providing services as required by the patient. In this particular case, the nurses need to use a graphic frame of reference in order to evaluate the desired behavior of the concerned patients. As the service user is suffering from dementia and is visually impaired, therefore it is very difficult for the nurses to communicate with the service user. The social model or the medical model needs to be followed by the health care representatives. The responsibilities of the nurses are to maintain boundaries as well as they should work within the confined therapeutic relationship. It is important for the care worker to maintain boundaries while treating the patients as it develops the desired relationship with the patients. Difference between the medical and the social model

The medical model generally defines the health of the patients free from diseases and, on the other hand, it provides information's related to influential and powerful discourse concerning health.  The social model highlights the changes made by the society and lifestyle of people.  It includes the determination of health by a different range of environmental, social and economic factors (Adams, 2008). The basic difference between the social model and the medical model is that social model focuses more on the environmental, social and the economic factors whereas,  the medical model is concentrated on a particular patient (Barone, 2008).  Sometimes it has been seen that the social model recognizes the social as well as the environmental factors and critically affect the health conditions and also produce inequities.  The particular case reveals the idea of the medical model in which the treatment focuses on the concerned patient, who is 60 years old and is depressed. The medical model particularly deals with a certain or specific disease and in this patient is suffering mainly from depression (Berkman and D'Ambruoso, 2006).  Moreover, the social model reflects the different factors related to environment and the economic health conditions of the patient, whereas the medical model serves well for the disease caused to the concerned patient.

Discussion of the role of care worker  

The social worker plays the most important role in evaluating and maintaining the good health of a patient. The social workers or the health care professionals should take care of monitoring and supplying resources to the concerned patients.  The professionals should have the desired skills such as supervisory skills, decision-making skills, stress management skills, effective communicational skills, and problem-solving skills related to the concerned patient. Moreover, the health care worker should have perfect knowledge about various diseases or the disease of a particular patient (Boylan, 2013).  Trustworthiness and consistency are two of the most important characteristics of medical health care workers in providing services to the patients. Suitable medication needs to be provided to the patient suffering from particular diseases and on the other hand, the care worker should not harm the religious beliefs and values of the concerned patients.

The report highlights the different aspects regarding the professional relationship of the health care professionals with that of the patients. It is evident from the study that reliable relationship has to be developed between the patient and the medical personnel to solve the persistent problems in an innovative way (Cruz-Cunha, Tavares, and Simoes, 2010). The healthy relationship makes the patient more comfortable with the health management personnel that help them to understand and discuss the issues in depth and, therefore, the reliable solution is achieved successfully.   The graphic frame is one of the most effective tools which are being utilized by the nurses to understand the behavior of the patient (Duffin, 2013). However, it can be stated that with the help of healthy relationship and appropriate social and medical model, the medical team has achieved the effective results and the overall health condition development, which is the primary of health care management, is achieved successfully.

Medical and social models both are different in their own perspectives because one provides reliable information regarding the diseases and important information about the current health condition of the patient (Hall and Bowie, 2013). On the other hand, the changes take place due to the positive influence of the societal activities conducted by the individuals to the patient is illustrated with the help of the social model. However, this particular model supports the positive thinking of patient health and well-being and also appropriate the cooperation from the society to overcome a particular mental health problems of individuals.

The responsibilities of a social worker play a crucial role in the development of a patient condition because the care and required health support system are provided and monitored by himself. With the help of the extraordinary skill and understanding for the patient problem, the reliable care is taken to development the current mental condition of the patient (Helal, Bose, and Li, 2012).  The required skills such as effective decision making and problems solving skills along with effective communication skills are the potential drives that help in understanding the problems in an innovative way and also help in providing reliable understanding to the patient family members. Appropriate medication and external activities impact should not affect the patient otherwise; it may create a barrier of health and well-being development (Howe, 2010). Therefore, it can be stated that with the help of innovative nursing care and suitable treatment produces the health management personnel achieve the reliable outcome, which simultaneously helps in community development. With the help of the skills and expertise of the nurse, the critical diseases such as mental diseases are reviewed innovatively and the nurses and health care personnel effectively maintain the required care and responsibilities.
