ASSESSMENT TASK 4: PROJECT

Task Summary:

There are two parts to this task:

§ Part A: You need to complete a feedback log about the implementation of the community and social participation plan.

§ Part B: you need to complete a set of written questions.

Part A: Implementation and review of community and social inclusion plan

What do I need in order to complete Part A of this assessment?

  • Client community and social inclusion plan
  • Access to suitable facilities, equipment and resources
  • Access to workplace policies and procedures
  • Access to the client and/or their family members and carer while participating in community/social participation activities
  • Your supervisor (or a person nominated by the supervisor)
  • Feedback logs (provided at the end of this task, one log for each occasion).

  • You will do this task in your workplace.

If your assessor identifies that you did not complete all parts of the feedback log in enough detail, or if you missed any sections, they will give you feedback and you will be asked to fix the parts that are wrong and resubmit.

Instructions:

For this task you are required to observe a client participating in strategies from their community and social inclusion plan over at least two occasions.

The details of the activity must include the who, what, when, where and why

Who – who was involved?

What – what was involved as part of the activity?

When – When did the activity occur?

Where – Where did the activity occur?

Why – How does it link back to the client’s needs, wants and preferences

You will need to record the outcomes of the client’s participation by completing a feedback and engagement log (provided) based on the strategies that you have observed. 

This record should be completed over at least two occasions.

Note: During implementation, your supervisor (or another person nominated by the supervisor) should be present and assist you in working out solutions to identified challenges or barriers as well as providing additional support and guidance for improving strategies. You will be supervised at all times during this assessment.

You must obtain permission from your supervisor and the client (or their family/carer) for the assessor to be present during this consultation. You will be supervised at all times during your work with the client.

Part B: Written questions
