Assessment Task 4: Assessment Task 4: Workplace Project – Working Collaboratively with Clients and their Support Networks

TASK SUMMARY

For this task you are required to consider three clients living with mental illness and/or AOD issues who you have worked collaboratively with to establish a basis for participation of family and carers in their individual recovery process and to facilitate ongoing participation in line with the person’s needs and wishes. You may have worked directly with each client or been involved with supporting a senior staff member or supervisor to facilitate the client’s recovery process.

Templates are provided for you to fill out and submit. It is highly recommended that you read through each section of the template so you know what you need to address. It may also give you a better idea of which clients or client groups to write about – as some may not address all of the criteria in the template. 

RESOURCES AND EQUIPMENT REQUIRED TO COMPLETE THIS TASK:

Personnel

  • Access to workplace personnel, such as colleagues and your supervisor.

  • Access to your workplace.
  • Access to any relevant workplace documentation to assist you with your work.

WHEN AND WHERE WILL THIS TASK BE COMPLETED?

  • You will need to complete this task in your workplace. You may fill out the templates provided in your own time or at work (where time allows).

WHAT HAPPENS IF I GET SOMETHING WRONG?

If your assessor determines that you have not completed all parts of the template in sufficient detail or your supervisor has not signed it, they will talk to you about resubmission. You will only have to redo the entries in the template that are identified as having issues or obtain sign-off from your supervisor; however, you will need to resubmit the template. Your assessor will discuss an appropriate timeframe for resubmission with you when you are provided with the outcome for this task.

STUDENT INSTRUCTIONS FOR TASK 4

For each client report, you should document the following: The steps and processes that you undertook to prepare for working with the client and their support network, the strategies that you used when working with the client and how you identified their needs, the strategies that you used when working with members of the care network and how you identified their roles and provided support, the collaborative approaches which you used to plan how support would be provided, the implementation and monitoring of the care network support, the communication skills that you used and your strategies for working safely.

Reflecting on your experience, complete a client report for each individual. Use the report template provided to help guide you in your response. Add/modify the space in the table where required.

Once you have completed your report, ask your supervisor to confirm that you supported this client in the manner described by asking them to sign the bottom of the report for each client.

Remember privacy and confidentiality principles when completing your report. Refer to clients using terms such as Client A, Client 1 and so on. Do not include any information in you report that could identify your client (such as location, names of places frequented by the client, schools attended, etc.)

Implementing and monitoring support

Discuss the implementation of the support plan. How did you maintain a relationship with members of the care network? What was the impact of the support on the client’s recovery? What went well? What would you have improved? Was there a positive impact on the client’s behaviour? Was there a negative impact on the client’s behaviour? How did you address this? How did you assist the client to identify their impact on members of the support network? As you stayed in touch with the members of the care network, did you need to provide additional support/change the way that you were providing support? Did the role of any of the care network members change over time? How did you continue to collaborate with the client and ensure that consents remained in place and current?

Communication

What were the communication strategies and approaches to practice that you used when working with this client and their support network? How did you clarify issues, provide support, collaborate with the client and members of their care network?

Workplace skills

Describe how you worked safely in your planning and interactions with this client and their care network. Did you communicate with the client about safety? What steps did you take to plan for an emergency/to manage risks and to ensure that you met your duty of care to the client and to members of their care network? Assessment Task 5: Workplace Observation

TASK SUMMARY

This task requires that you are observed in the workplace. You will be observed in the workplace environment undertaking a range of activities that relate to working collaboratively with a person with mental illness and/or AOD issues to establish a basis for participation of family and carers in their individual recovery process.

The purpose of a workplace observation is to confirm that you can apply your skills and knowledge to real-world situations, dealing with real people and real workplace processes, following policies and procedures, and handling multiple work activities.

RESOURCES AND EQUIPMENT REQUIRED TO COMPLETE THIS TASK:

Personnel

  • Your workplace supervisor.
  • Your assessor.
  • Access to a computer, printer, Internet and email software (if required).
  • Access to Microsoft Word (or a similar program).
  • Access to a workplace environment.
  • Access to organisational policy and procedures.

WHEN AND WHERE WILL THIS TASK BE COMPLETED?

Your assessor will provide you with a time and place for your assessment. 

WHAT HAPPENS IF I GET SOMETHING WRONG?

If your assessor identifies that you did not correctly or adequately demonstrate the skills and knowledge required of this task, you will be given feedback on areas for improvement.

Your assessor will provide you with guidance as to whether you need to: 

  • redo the observation at a suitable time after you have considered their feedback ? undertake further learning and redo the observation at a later date.

Your assessor will provide details about the date, time and location of reassessment. 

STUDENT INSTRUCTIONS FOR TASK 5

Your assessor will visit your workplace and observe your work performance as you undertake a range of activities that relate to working collaboratively with a person with mental illness and/or AOD issues to establish a basis for participation of family and carers in their individual recovery process.

During the observation, your assessor will be looking to see that you can:

  • work collaboratively with the client to identify a support network
  • work collaboratively with a member of the client’s support network ? work safely.

Your assessor will also ask you some verbal questions during the workplace observation. The questions will relate to the following topics:

  • work role boundaries
  • consents
  • mandatory reporting
  • privacy, confidentiality and disclosure
  • records management
  • rights and responsibilities ? reflective practice.

WHAT MUST BE SUBMITTED FOR TASK 5:

You are not required to submit anything for this task.
